Chronic Disease Management Plan Psychology Rebates
A Chronic Disease Management Plan (CDM) allows individuals with a long-term health condition to access Medicare-rebated psychology sessions as part of their ongoing care.
This program is available to people managing a chronic condition — physical or mental — and can be used to access psychological support under Medicare.
🩺 What Is a Chronic Disease Management Plan?
A Chronic Disease Management Plan (also known as a Team Care Arrangement) is prepared by your GP to help coordinate care for individuals with a chronic medical condition. This includes referrals to allied health professionals like psychologists.
Your GP can refer you for up to 5 rebated allied health sessions per calendar year under the CDM plan. These sessions can include psychology, occupational therapy, dietetics, physiotherapy, and more.
💡 Who Is Eligible for a CDM Psychology Referral?
To be eligible for CDM-funded psychology services, you must:
- Have a chronic medical condition (lasting 6 months or more)
- Be under a GP Management Plan
- Have a Team Care Arrangement in place (organised by your GP)
Examples of conditions that may be eligible:
- Chronic pain
- Diabetes
- Anxiety or depression
- Autism or ADHD
- Eating disorders
- Fibromyalgia or fatigue-related conditions
- Long-term injury recovery
Psychology Rebates with a Chronic Disease Management Plan

Medicare rebates may be available for some psychology sessions with a valid referral or eligible Medicare plan.
As of 1 July 2026, current Medicare rebates for CDM services include:
- Clinical Psychologists: $63.40 per eligible session.
- Registered Psychologists: $63.40 per eligible session.
- Provisional Psychologists: Medicare rebates are not available.
Rebates are subject to Medicare eligibility, referral requirements, and session limits. Medicare rebates are processed after each appointment has been paid in full.
You can claim rebates for up to 5 sessions per year. These are separate from the Better Access Mental Health Care Plan and can be used alongside it, where appropriate.

- Book an appointment with your GP
- Request a Chronic Disease Management Plan with a Team Care Arrangement
- Ask for a referral to our psychologists (you’ll need a signed referral form)
